Belhassen Tachycardia in a 19-Month-Old Child

A 19-month-old girl presented with tachycardia, at a rate of 300 beats per minute and a 2-day history of increased irritability without fever in the setting of upper respiratory infection symptoms. Her initial electrocardiogram revealed tachycardia and a mildly prolonged QRS duration (Figure). Her transthoracic echocardiogram showed normal intracardiac anatomy without a pericardial effusion. Vagal maneuvers, multiple doses of adenosine, and synchronized cardioversion were attempted without success or slowing.
